Fees For Testing

 A.  Most of our patients have insurance coverage for standard fertility screening tests.  In many cases these tests have been ordered & obtained by your primary physician or Ob-Gyn prior to your referral to us for treatment.  The fees will depend on the test & the laboratory, radiologist or other facility where it is performed.  Payment is made direct to the provider of services.
 
 B.  For outside consultations/treatments
The fees for Psychological therapy, acupuncture, legal consultations, surrogacy agencies, egg donor agencies will be determined on an individual basis & paid directly to the provider.
 
C. In House Tests/Services

Our cycle success guarantee program, if you are eligible, is $17,600; including UNLIMITED Frozen Embryo Transfer cycle, if embryos are available.  This program will return a significant percentage of the fees based on your age if no pregnancy occurs.

Extra fees for medications may be $3-6000
Annual Storage fee =$350
Anesthesia fees =$470
fees for outside counseling, legal consultation, etc

8.Gestational Surrogate IVF Cycle =$17,605
Includes ICSI, AH & Embryo cryopreservation
Extra fees to pay Agencies for surrogate, psychological assessment, legal counsel & medications

9.Egg Thaw/IVF/Transfer Cycle =$6800

10.Combined Egg Donor & Gestational Surrogate IVF Cycle = $21,725 (including ICSI, A.H. & Embryo cryopreservation.  Extra costs:  $3-6,000 for meds, fees for agencies, Psychological evaluation Compensation to egg donor & the surrogate, etc.

Surgical Fees; vary based on the exact type and difficulty of the specific operation you may need.                 Ranging from approximately $750 to $8,500 ranging from D&C's to complex Laparoscopy or Hystoroscopy.

12.  Hysteroscopy to diagnose Polyps, Fibroids, Septums

13.  Laparoscopy to diagnose & lazer Endometriosis, removal of Fibroids, Fallopian Tube Repair & Fimbrioplasty.
